Make a Report
If you, or someone you know, has experienced conduct that may constitute discrimination, harassment (including sexual harassment), retaliation, dating violence, domestic violence, sexual exploitation or stalking, it is important to report the information to CPHD.

CPHD will review the report to determine whether the conduct should be addressed under the Interim CSU Nondiscrimination Policy. Our office will typically reach out to the impacted party (the person who experienced the harm) to offer support and schedule an intake meeting. CPHD may occassionally receive reports that are best addressed by other campus departments. In these cases, CPHD will make the necessary referral(s) to ensure the concerns are addressed by the appropriate campus administrator(s).
Privacy and Confidentiality
CPHD will make every effort to safeguard the impacted party’s privacy and allow them to control what happens after a report is made. However, while we are committed to providing individuals with as much autonomy as possible, we are not a confidential resource. We may be required to share information in reports with other SDSU administrators to help fully assess the situation and make determinations related to campus safety.

Anonymous Reporting
Reports may be submitted anonymously. However, it is important to know that completing the form anonymously may limit CPHD's ability to respond to the report.
